If you cannot find a form you are looking for use the search bar above the table in the top right.
Many of these forms can be completed online and then printed out and filed with the Summit County Clerk of Courts office. Others must be printed and completed manually.
YOU MUST “FLATTEN” YOUR PDF BEFORE SUBMITING TO CLERK OF COURTS
Follow the steps below to “flatten” a completed PDF fillable form:
- Open fillable form.
- Add appropriate data.
- Select Print. (NOTE: if document opens in Adobe, Select File and select Print.)
- Select the PDF printer. (The Adobe PDF printer is installed automatically with Adobe Acrobat. Numerous free PDF printer drivers are available for download from the Internet.)
- Select OK.
- Specify location to save the printed, “flattened” version of the form.
- Select Save.
